Please print out and complete the following forms prior to your visit and bring them with you to your first appointment. If you do not have access to a printer, please let our staff know when you come for your appointment.

What to bring to the appointment:

• List of prescriptions and/or over-the-counter medications, including dose and frequency
• Information about the patient's medical and surgical history
• Relevant records

We do NOT take insurance or payment for any studies that you participate in. They are completely free.
